fix(canvas/settings): add focus-visible to secrets-tab refresh button (WCAG 2.4.7)

The Refresh button inside the SecretsTab error state had no focus ring defined in CSS. Without it, keyboard-only users cannot determine which element has focus on that error screen. Co-Authored-By: Claude Opus 4.7 <noreply@anthropic.com>
